Incidents
Incidents are events that actually happened. For example, the car hit the fence.
Hazards
Hazards are something that have the potential to turn into an Incident. For example, there is a large pot hole in the road which may cause significant damage to a vehicle if driven into.
Near Misses
Near misses are events that nearly happened. For example, the ship's engine stopped half way out the channel and the ship began to drift dangerously, but the engine restarted in time and no Incident occurred.
Observations
Observations are more behavioural-based, and are recorded as an FYI only, and generally do not require any actions.
Inspections/Audits
Inspections and audits are verifications of systems, processes or things.
